payment option problem

this issue is still with me.i provided every thing (payment option ) required.
it takes time to review then give me another message.now they said "put the name in name box and surname in surname box".meanwhile,I had done that before but reviewer is not understanding . but even then i changed again .I am depressed because of this issue .i don't know when it will be solve. please solve this issue CG authority
