What ever that works best for you, is the best approach.
For me it is (most often) gathering reference pictures, maybe drawing some sketches (I should do that more often), then I create a rough shape of the model using primitives only, so I get the hold of the proportions. Adding more and more details. Cleaning up in geometry and so on. Texturing is my favorite part.
